• HeartVoice App account for home monitoring of blood pressure, heart beat, blood glucose,

        oxygen saturation and more.

Eligibility Criteria:

• Singaporeans/ PRs 40 years or older; and/or

• Singaporeans/ PR 18-39 years, at higher risk of chronic conditions as identified by Pre-

        Screening Questionnaire (DRA)
